To document and report student misconduct, or to terminate a student’s enrollment in a faculty-led Study-in-America/Off-Campus Program, download and complete the Student Misconduct Report. This Word document includes two forms, the Initial Student Misconduct Report in a Faculty-led Study-in-America/Off-Campus Program and the Termination of Student from a Faculty-led Study-in-America/Off-Campus Program. The first form is a way to document and provide a warning to students whose behavior is deemed inappropriate by the Academic Program Director. Use the second form if termination of a student is necessary.
The first form must be printed and signed by the Academic Program Director, witness(es), and the student. With the second form, include documentation supporting the decision to terminate the student’s participation. Copies of the completed documents and all supporting documentation may be scanned and e-mailed, faxed, or mailed to the Office of Distance and Extended Learning (ODEL). Please retain the originals, and submit them to ODEL upon your return to campus.
